Cheryl Matter, PhD, Vice President, Population Health, Careforth

At Careforth Cheryl Matter leads a team responsible for developing and implementing sustainable and scalable models of care that improve the health, well-being and experience of patients and their caregivers. Cheryl has more than 15 years of experience advancing innovative healthcare solutions focused on addressing the needs and barriers of our most vulnerable and complex individuals. Most recently, Cheryl served as the VP of Population Health at Lifesprk, a senior-focused home and community service organization where she was accountable for the development, tech-enablement and integration of offerings that include home-based primary care, care management, hospice, risk adjustment and partner management. Prior to Lifesprk, Cheryl led the development of a tech-enabled community-based palliative care service for Livio Health, a BCBS MN sister organization and held a variety of clinical, business and leadership roles within emerging companies, accelerators, and industry support organizations. Cheryl has a PhD in Molecular and Medical Pharmacology from UCLA
