Within the framework of an interdisciplinary, concentrated group offer, arrangements will be made for patients with chronic health problems to receive highly competent help to formulate tailored treatment goals where they take part in their own health change project at all stages. The concentrated offer is carried out within a week, with online follow-up for one year. HiH opens in autumn 2020.
